Please sign in using your last (family) name and the ID number listed at the top of the Annual Meeting & Courses registration confirmation form. The system will then ask you to indicate the sessions you attended, to complete evaluation forms for each of those sessions, and will generate a PDF certificate which may be printed or saved to your computer.
